YOUR INITIAL VISIT

Your first appointment with Dr.Alavi, your Santa Rosa family dentist, will consist of a consultation explaining your diagnosis and treatment based on a complete medical and dental history, clinical examination, and required dental x-rays. If your initial visit is an emergency, all effort will be made to help alleviate any discomfort that you may be experiencing.

Please be prepared to fill out a medical history form and bring a list of all your current medications.

NOTE: Patients under 18 must be accompanied by a parent or guardian for their initial visit.
